NEW CONSULTATIONS: We ask patients to arrive 15 minutes prior to their appointment to complete the new patient paperwork. If we need to obtain medical records, a signed Records Release is required before the scheduled appointment.

IMPORTANT: Please remember to bring actual medication bottles, not just a list.

NOTE: Due to patient emergencies, scheduled appointments may be delayed or rescheduled. While we do our best to notify patients ahead of time, we cannot predict emergency situations. Thank you for your understanding.

POST-OP APPOINTMENTS: It is the patient's responsibility to contact our office to schedule follow-up appointments after being discharged from the hospital or nursing center.
